USMLE Step 3 Forms and Fees

Application – $830

USMLE Step 3 Application – The Step 3 application fee is non-refundable and non-transferable from one eligibility period to another or from one application to another. Your application fee must be paid by American Express, Master Card or Visa.

2016 Fees – Effective Date for 2016 Fees

$830 ( 1. Three month eligibility periods that end in 2016.

2. The first eligibility period in which the 2016 fee will apply is November 1, 2015 -January 31, 2016.

3. The last eligibility period in which the 2016 fee will apply is October 1, 2016 -December 31, 2016. )

Certification of Identity (CID ) form – A CID is required for all Step 3 applications unless one is already on file with FSMB and has not expired. CID Forms prior Step 3 applications are valid 5 years from the date of the applicants signature.

Name Change/Correction  – Name change.Correction Authorization Form – Evidence of the change/correction must also be submitted.

Test Accommodations – Visit the Test Accommodations page on the USMLe website to download the forms to:

– Request new test accommodations or to request a change to accommodations received previously

– Request the same accommodations for Step 3 that you received on Step 1 or Step 2

Eligibility Extension ($70) – If you need to extend your eligibility period, you must submit an eligibility extension request and $70 processing fee. If your request is approved, a one time contiguous 3 month extension will be granted ( Note : “ Contiguous “ means that the start date of the eligibility extension immediately follows the end date of our eligibility period). Request can only be submitted after your scheduling permit is available and must be received no later than 25 days after the end of your original eligibility period. Requesting an extension of your eligibility period does no cancel a scheduled exam appointment. If you have a scheduled testing appointment during your original eligibility period and need to reschedule the appointment for an extended eligibility period, you must cancel or reschedule with Prometric. A fee may be changed if you change your appointment depending upon how much notice you provide when making gate change. Score Recheck ($80) Form – A recheck of your Step 3 Score will be done upon submission of the recheck form and $80 fee. Please note that, for all USMLE Steps, Standard Procedures ensure that the scores reported for you accurately reflect your performance. A change in score based on a recheck is an extremely remote possibility.
